ABSTRACT

This study aims at reporting the indigenous knowledge of the medicinal flora from the inhabitants of surroundings of the World's largest artificial planted forest "Changa Manga", Pakistan. Data were collected by direct interviews and group meetings from 81 inhabitants including 32 local healers having information regarding the use of indigenous medicinal plants over a period of one year. Different statistical tools were applied to analyze the data including Frequency citation (FC), Relative frequency citation (RFC), Use Value, Factor of informants consensus and fidelity level. This study reported 73 plant species belonging to 37 plant families and 46 genera. The majority of plant species belong to compositae family. The most commonly used medicinal plants were P. hysterophorus L., P. dactylifera L., S. indicum L, P. harmala L., P. emblica L., and A. indica A.Juss. The greatest number of species was used to cure gastrointestinal disorders. The highest fidelity level (68.18%) was of E. helioscopia to cure gastrointestinal disorders. Maximum fresh uses (17) were reported by C. dactylon (L.) Pars. While the highest number of species reporting fresh uses in similar number was 13. In this study, five novel plants are being reported for the first time in Pakistan for their ethnomedicinal worth. Our data reflect unique usage of the medicinal plants in the study area. The statistical tools used in the study proved useful in pointing the most important and disease category specific plants. High use value plant and the new reported medicinal plants might prove an important source of the isolation of pharmacologically active compounds.


Este estudo tem como objetivo relatar o conhecimento indígena sobre a flora medicinal dos habitantes do entorno da maior floresta artificial plantada do mundo, a Changa Manga, no Paquistão. Os dados foram coletados por meio de entrevistas diretas e reuniões em grupo de 81 habitantes, incluindo 32 curandeiros locais, com informações sobre o uso de plantas medicinais indígenas durante o período de um ano. Diferentes ferramentas estatísticas foram aplicadas para analisar os dados, incluindo citação de frequência (FC), citação de frequência relativa (RFC), valor de uso, fator de consenso dos informantes e nível de fidelidade. Este estudo relatou 73 espécies de plantas pertencentes a 37 famílias de plantas e 46 gêneros. A maioria das espécies de plantas pertence à família Compositae. As plantas medicinais mais utilizadas foram P. hysterophorus L., P. dactylifera L., S. indicum L., P. harmala L., P. emblica L. e A. indica A. Juss. O maior número de espécies foi usado para curar distúrbios gastrointestinais. O maior nível de fidelidade (68,18%) foi de E. helioscopia para cura de distúrbios gastrointestinais. Os usos máximos em fresco (17) foram relatados por C. dactylon (L.) Pars. enquanto o maior número de espécies relatando usos frescos em número semelhante foi de 13. Neste estudo, cinco novas plantas estão sendo relatadas pela primeira vez no Paquistão por seu valor etnomedicinal. Nossos dados refletem o uso exclusivo das plantas medicinais na área de estudo. As ferramentas estatísticas utilizadas no estudo mostraram-se úteis para apontar as plantas mais importantes e específicas da categoria de doença. Plantas de alto valor de uso e as novas plantas medicinais relatadas podem ser uma importante fonte de isolamento de compostos farmacologicamente ativos.

Feral dogs are well-organized hunters of ungulates in many parts of the world, causing great damage to wildlife populations and ultimately to the ecosystem. In Pakistan, the impacts of feral dogs on the wildlife have not been documented yet. In a period of fifteen years (2006-2020), feral dogs have killed hundreds of threatened markhor in Chitral gol national park (CGNP), Pakistan. Despite direct predation other impacts including disturbance and competition with other natural predators, could compromise conservation and management efforts. The population of feral dogs seems to have been increased with the increase of dumping sites by communities. Our findings suggest that there are pressing needs of controlling the feral dogs population and eradicating them from the core zone of CGNP and surrounding buffer communities. Conventional culling of dogs should be coupled with modern techniques like castration and sterilization. Communities should be educated regarding the clean environment, proper disposal of home wastes and, biodiversity conservation.


Os cães ferozes são caçadores bem organizados de ungulados em muitas partes do mundo, causando grandes danos à população de animais selvagens e, em última instância, ao ecossistema. No Paquistão, os impactos dos cães selvagens na vida selvagem ainda não foram documentados. Em um período de 15 anos (2006-2020), cães selvagens mataram centenas de markhor ameaçados no parque nacional Chitral gol (CGNP), Paquistão. Apesar da predação direta, outros impactos, incluindo distúrbios e competições com outros predadores naturais podem comprometer os esforços de conservação e manejo. A população de cães selvagens parece ter aumentado com o aumento dos locais de despejo pelas comunidades. Nossas descobertas sugerem que há necessidades urgentes para controlar a população de cães selvagens e erradicá-los da zona central do CGNP e das comunidades-tampão vizinhas. A total of 86 species belonging to 39 vascular plant families, 33 genera were documented as medicinally important. Family Asteraceae was observed as the dominant family among all the families with 10 species, the leaf was the most used parts and decoction 36% was the most preferred preparation type. Herb was the predominant life form (67%). The maximum UV (0.92) was demonstrated by J. adhatoda L. species, while A. sativum L. shows maximum RFC (0.58), the highest ICF value represented by diarrhea and dermatitis 0.92, and high FL value is recorded 100%. According to our collections, wild species were 45%, invasive species were 38% and cultivated 17% recorded, dicots species were recorded more 81%. Seven 7 medicinal species is being economically important and export to the local and international market of the world, whereas P. integrima L. species were the most exported species according to the local dealers. The investigated area is rural and the local people depend on the area's plants for their health needs, and other uses like a vegetable, fuelwood, fodder, etc. The current result of [...].

Nosso estudo teve como objetivo inventariar medicamentos de plantas locais, documentar seus usos e avaliar seu valor de mercado em 2015-2018 durante as temporadas de primavera, verão e inverno. Foram feitas 15 viagens, 5 em cada temporada. Foram realizadas entrevistas semiestruturadas com 165 moradores na faixa etária de 20 a 80 anos, com dados analisados por meio de frequência relativa de citação (RFC), valor de uso (UV), nível de fidelidade (FL), fator de consenso de informantes (CIF), e o índice de Jaccard (JI) para encontrar as espécies utilizadas mais frequentes e conhecidas na área. ABSTRACT

In highly endemic countries like India, where tuberculosis (TB) and leprosy infection may coexist, screening the other disease before initiating treatment is important to prevent Rifampicin resistance since both diseases are treated with and sensitive to Rifampicin. Here, we report a leprosy case involving the unmasking of leprosy in a treated patient with Pulmonary TB. In this case, a high index of suspicion of Erythema Nodosum Leprosum (ENL) in a patient with no history of leprosy disease or treatment with anti-leprosy drugs was observed. He, however, had a history of taking anti-tuberculous medicine 1.5 years earlier. This case report also acknowledges the physician’s prompt referral of this patient to a dermatologist. Taking a detailed family history and screening helped us diagnose leprosy in the patient’s daughter. It also emphasises the atypical presentation of leprosy, which (although described in textbooks) is being reported here.

ABSTRACT

Introduction: First case of COVID-19 in the district was reported on 19th March 2020, thereby increas-ing exponentially, presenting with various symptoms to having few or no symptoms, posing challenge to prevent disease transmission by being a significant source of infection. This study conducted to assess relationship between COVID-19 infection within blood groups and burden of reported cases in the dis-trict to seropositivity among donors. Methodology: This is a prospective observational study from second largest blood bank of south Guja-rat, catering to a population of seven million, 4916 donors from the month of August 2020 to May 2021 participated in the study. Observations: Seropositivity was found to be 54.6%, with higher prevalence (68.63%) among female compared to male (54.45%), there was no statistical difference among age groups & RH groups, blood group AB was found to have highest seropositivity followed by A, B and O group. First time donors sero-positivity was higher compared to frequent donors among the study participants. Seropositivity among donors was positively corelated with RTPCR Positivity rate reported in the district. Conclusions: Looking at similar trend of active surveillance and its correlation to blood bank positivity, we urge policy makers to strengthen hospital-based surveillance for real time insights.

ABSTRACT

Background: To increase research productivity among post-graduate (PG) students, there is inclusion of research dissertation or thesis in their academic curriculum. However, thesis or research project during the residency remains is the last priorities due to high demands on the PG student’s energy and time in terms of patient care. There are a very few such studies conducted in our nation pertaining to perception of PG students toward thesis. Aim and Objectives: This study is to assess the attitude and perception of medical PG residents toward research dissertation or thesis during residency training. Materials and Methods: This was an observational, cross-sectional study with the 2nd and 3rd year MS/MD PG students as participants. Study tool was pre-validated questionnaire consisting 40 questions under following domains: University regulations (5), attitude toward thesis (4), perception toward benefits of thesis (15), perception toward obstacles of thesis (13), and practice in regard to thesis (3). A study was initiated after approval of Institutional Ethics Committee. After obtaining written informed consent, a questionnaire was given to each student and was collected after 25–30 min. Confidentiality of responses was ensured by maintaining anonymity of responders. Data were entered into Excel and a descriptive analysis was done. Results: Of these 55 pre/paraclinical and 70 clinical PG students (n = 125) returned completely filled questionnaire. The mean age of the students was 27.40 ± 2.20 and was in the 2nd or 3rd year of the residency. 256% participants were from clinical whereas 44% were from pre/paraclinical branches. The majority of postgraduate students preferred observational studies (80%) over interventional study for thesis (19.2%). 61.6% PG students felt that thesis should be mandatory. Majority of students agreed to benefit of thesis which helps them to develop skills such as literature search (92.8%), data processing (84%), statistical analysis (73.6%), interpretation of results, and discussion (80.8%). PG students felt that there are few obstacles in their thesis such as inadequate lab facilities (66.4%), inadequate study tools (62.4%), and incompetent technical staff (55.2%). Conclusion: A number of barriers such as lack time, funding, and poor availability of research facilities for conduct of research need to be addressed to further enhance PG student’s positive attitude toward thesis.

ABSTRACT

Introduction: In Western population, a threshold of 5 drinks per day may exist for alcohol to increase pancreatitis risk. Given ethnic differences in alcohol metabolism, we examined the associations between smoking, alcohol, and pancreatitis in Western Indians. Methods: A case control study was conducted in a surgery department of a hospital in western India. In-formation on drinking was collected by in-person interview. Baseline characteristics and alcohol con-sumption was compared between cases of pancreatitis and control (without pancreatitis). Results: Baseline characteristics of cases and control are Among 4% of the cases and 2% of the control, bile stone was found to be present and this difference was also statistically not significant. Alcohol use was associated with pancreatitis in a dose-dependent way. Those who were taking heavy amount of al-cohol had more than five and half-time risk of developing pancreatitis compared to those who are not taking alcohol. Conclusions: Indians are more prone to alcohol-related pancreatitis than Westerners, and alcohol con-sumption is the leading cause of pancreatitis in India.

Background: Spirometryis the GOLD standard for the diagnosis of COPD. This test also grades the severity of the disease which helps in the management of patients.Many times Spirometry is not available in remote areas. In these situations 6MWT can be done. 6 minute walk test (6MWT) is a simple test to assess patient’s functional capacity (ability for day to day activities).Material And Methods:A prospective study was conducted in the department of Respiratory Medicine, AMC MET MEDICAL COLLEGE, L.G. HOSPITAL, AHMEDABAD for 6 months from date of Confirmation of IRB.The patient who was a confirmed and stable case of COPD and who gave consent was taken for the study.Conclusion:The 6MWT plays a major role in measuring the functional status and disease tolerance of COPD patients.Also, it is observed that as severity of FEV1 increses 6MWD decreses.This suggests that in the absence of spirometry 6MWT is a reasonable tool for the assessment of disease severity and functional status in COPD Patients.

Background: Head and Neck cancer (HNC) is a recognized major public health concern all over the world Objectives: To study age and gender predilection and clinical profile of Head & Neck Cancers Materials and Methods: This was a retrospective study including 21121 patients with Head & Neck Cancer presented to tertiary care centre, Ahmadabad. Patients' details of age, sex, site of carcinoma, stage at presentation and Outcome were recorded. The data was analyzed using MS office excel 2010 and Epi info software version 7.0. Result: Male to female ratio was 4.8:1. The most common site of presentation of tumour was in cheek mucosa (29.01 %). On comparison of different anatomical sites, majority of patients were presented in locally advanced stage. Significant association (p<0.05) was found between presentation of disease and literacy status. Overall 5 - year survival rate was 60.5% � 1.6% when disease was localized. Conclusion: Majority of patients diagnosed in locally advanced stage. Strategies for the early diagnosis and prevention of Head & Neck cancer must be undertaken by government to overcome this situation.

Background:Febrile seizures are among the leading cause of pediatric emergency hospital admission and affect 2% to 5% of pediatric population1,2 and are by far the most common type of seizure in childhood.Various risk factors are said to play a role in aetiology of FS are gender, developmental delay, maternal history of smoking, family history, bacterial and viral infections, certain vaccinations and iron deficiency, low serum sodium at the time of presentation. Preventive measure to remove such risk factor could lead to lower the incidence of febrile seizure.Objective: To study occurrence of predictive risk factors like gender, age, low birth weight in children with febrile seizure. To study fever and seizure pattern in above children. To observe positive family history pertaining to febrile seizure. Material And Methods:An observational prospective study was conducted in the tertiary care hospital over a 2 year period. Children aged 6 months to 60 months diagnosed as febrile seizure and admitted at pediatric ward were included in the study. A total of 69 patients were enrolled in the study. All information was collected from parents and indoor case sheets after taking verbal consent. Data was entered in Microsoft excel and analysis was carried out using SPSS version 21. Result:Gender wise amale predominance was seen. Youngerage group isa risk factor. Majority of children (92.8%) presented with simple FS with seizure duration less than 5 minutes. Majority (82.6 %) had fever for < 24 hour prior to FS. Positive family history of FS is observed in 20.2% children. Conclusion:Identification of predictive risk factor will help clinician to educate and counsel parents regarding seizure recurrence, prophylactic use of antipyretic and measures during seizure activity.

Erythema ab igne is a characteristic reticular telangiectatic and pigmented dermatosis, resulting from repeated or prolonged exposure to heat or infrared radiation insufficient to cause burns. Occupational history has played a pivotal role in dermatological diagnosis, and its importance cannot be overstated. It could lead to the provisional or final diagnosis in many instances.Over the course of time, the site and cause of erythema ab igne has evolved. From commonly being found on knees shins and palms due to tapnas (a practice in Indian villages to sit in front of fire); the disease is now observed on other sites such as on thighs and trunk after the discovery of heating pads and laptops.In thisparticular case, a 26-year-old male presented with erythematous to light brown patches on his right arm. The site of presentation was quite unusual, thus making occupational history the key to diagnosis.

ABSTRACT

We report a rare case of Pseudomonas stutzeri endophthalmitis in an immunocompetent individual along with the review of the literature. A 39-year-old healthy lady presented with sudden painful loss of vision in her right eye. She was diagnosed with postcataract surgery acute endophthalmitis and underwent vitrectomy, intraocular lens explantation and intravitreal antibiotics. P. stutzeri was isolated from vitreous. Though the infection was controlled, the anatomy of the eye could not be salvaged and the right eye became phthisical. P. stutzeri is a rare cause of endophthalmitis with usually poor outcomes.
